Paradigms suggests an analyst of systems failures in industry and educational institutions are the root of many problems. A paradigm is a "set form" which is so familiar as to prevent us from seeing what is actually before us. By way of example he thumbs through a deck of cards very quickly, displaying the cards to the viewer. The first time through one's reaction is one of nonchalance. The second time the cards are presented slightly slower. There still seems to be nothing wrong but the viewer, not knowing to look for something out of order, is left with a vaguely disquieted feeling.
